Balance

I find the Index far more comfortable after filling the gap in the back with the equivalent of a somewhat curved equivalent to a roll of quarters. The result is close to the head, meaning the short lever arm relative to the head makes it follow the head easily. I'm using thin velcro straps to keep it in place, but lots of methods could work.

The result is that downward pressure on the nose and face is eliminated, and much lighter pressure can be used for the lighter task of keeping the headset in place.

Ventilation

I bought an after-market replacement for the face bracket and pad, purely to keep the original one pristine, and cut ventilation divots/slots(?) into the plastic part, especially in the top of it, and ideally in the sides as well. I wear the result without the nose guard (since my Index is balanced), with the result that the warmth of my face and movement combine to move air upwards from the nose area through the space in front of the eyes and out through the gaps cut into the top.

The result is that fogging is entirely eliminated, even when using the KAT Walk C2+.

One caveat is that the openings do let in light - since I usually use VR in a dim room (lit in red or purple), and even in a light room generally don't notice the extra light anyway, this doesn't trouble me. Ideally the ventilation holes would be placed so that there wouldn't be a direct light path through them by the vendors, but somehow almost no vendor has considered fogging a serious problem, getting hung up in blocking light instead.

Controller Power

I often spend long sessions in VR, making a cabled headset and controllers mandatory. In the case of the Valve Index controllers, On each controller, I've added a very short USB cable with a 90° coupler, velcroed to the outer hand guard. This isn't perfect, since it interferes with adding one of those magnetic fulcrums to quickly socket to a magnetic coupler on the chair for pinned joystick use, but it does dramatically reduce wear from replugging USB on the controller itself.
