So you're starting a Permadeath run.

The first thing to know is that you're not just trying to survive the game, but also the game's bugs.

Anything that kills you over time essentially gives you the opporunity to load from a save before your health reaches zero, meaning that the extreme hazards are those that can kill you before you can react. So, with that in mind:

Non hazards: Sentinels, space combat. These all give you oodles of time to decide if you need to reload before getting killed.

Vanishingly rare actual hazards: Being trapped in a high damage area or player-created trap. Mitigation: turn off network play (multiplayer). Avoid teleporting to other player's bases. Avoid short-range teleporters built by other players. Avoid teleporting to "orbital" bases. Don't build "orbital" bases (I have an orbital base that will kill you in seconds from asphyxiation if you stand on top of it).

Common game-bug hazards: Incomplete geometry after teleporting (much less common now), getting shoved out of geometry by players, doors, etc (also less common now), falling through the surface (now returns you to the surface), falling out of the Anomaly, getting your spaceship stuck inside of another spaceship, freighter, or other space object (rare). Mitigation: be wary of these hazards, and be ready to load from your save if you start falling or get stuck somewhere. Don't build your own bases over dropoffs or hazards.

Stupid hazards: Weapons that let you kill yourself. Shooting ship missiles at an object too close to you. Mitigation: Don't install them in your multitools. At all. Don't install rocket launchers on your ships.

✠✠✠ Max hazard ✠✠✠: Animals. Especially animals in tunnels, but any pit in the surface is a high-risk area, which includes those you create while mining ore. They can kill you unbelievably quickly. Animals will sometimes teleport into tunnels by glitching through geometry (or whatever), meaning you can't even trust an empty tunnel to stay empty, especially if it's close to the surface. Mitigation: Avoid tunnelling on planets with lots of fauna or hostile fauna, be wary of pits, caves, etc (where, unlike tunnels, at least you can get into the air), stand on elevated objects or your ship while mining.
